COVID-19 Vaccination Policy

All faculty and staff (including contract workers) who are employed at HMC and for whom the COVID-19 vaccine is approved/authorized by the FDA, must be vaccinated against COVID-19 and show proof of vaccination, unless an exemption from this policy has been granted as an accommodation or otherwise.

All persons covered under this policy must provide proof of full vaccination (both doses for a two-dose vaccine) or submit a request for an accommodation or exemption to the Office of Human Resources, in a manner acceptable to HMC upon date of hire.

COVID-19 Surveillance Testing Policy

Faculty and staff coming to campus regularly will be required to participate in surveillance testing for COVID-19 on a regular basis as a condition of coming on campus. The testing requirement is in addition to the daily symptom screening requirement and compliance with College policies described in the Guide for Faculty, Staff and Student Employees Returning to Campus.

Faculty or staff members who are approved to work on campus and who are not fully vaccinated, including those requesting an accommodation or an exemption to the vaccine requirement (e.g. medical or religious based on an individual’s sincerely held religious belief, practice, or observance), will be required to test at least twice a week through Student Health Services.

Please complete and return both consent form to hr@hmc.edu.  Please allow 24-48 hours once you submit your forms before you begin testing.
