All deadlines for forms and documents are listed by due date and should be returned to the Dean of Students office. Please allow several days to get these forms checked into on our Received Checklist and distributed to the correct department. The deadlines are important: simply mark the 15th of June, 1st of August and 10th of August on your calendar as important deadlines. Note the deadline for placement forms is the 22nd of July. The 25th of August, is the deadline for the Final High School Transcript. The Housing Agreement is TBA after the students arrive. - Roommate Matching Form
The Roommate Matching form is essential for you to complete thoroughly and honestly (no parents peeking). Only a few people read the forms, and they will be kept confidential. The orientation directors will very carefully try to make a workable match of you and a roommate as well as you and a dorm.

- Student & Family Information Form
The Student & Family Information form collects the same information previously collected by the Room & Board, Parent and Emergency Data Information forms. The form is relatively short and should not take too much time to complete. It asks you for general information, family contact information, at least one emergency contact and meal plan choice.

- Tuition Payment Agreement (PDF)
All incoming students are required to designate a tuition payment method. Please follow the link to view payment options. Please make sure to initial your payment choice and sign the form. A scanned or faxed copy is considered preliminary information only, and a hard copy sent in by mail is necessary.
Skip over the Student ID number portion since you will not have that until you arrive on campus.

- Photos
Each year, photos of new students are published in the Student Handbook. If you have not already submitted your pictures, please send one digital copy (or two hard copies) of a color or black-and-white photo of yourself.
GUIDELINES FOR SUBMITTING YOUR PHOTO:
- Resolution: 300 pixels minimum at final size for best results.
- Image size should be a HEADSHOT and at least: 2.5 x 3 inches. (If your photos are 72 dpi, make sure they are at least 7.5" x 9".)
- Please save your photo with your LASTNAME_FIRSTNAME as the filename with the .jpeg extension.
- Head and shoulder photos (rather than entire body shots) are preferred.
- Photos should have a clear view of only the student (i.e., no pictures where your face is right next to someone else) suitable for cropping/shrinking to a wallet-sized portrait.
- If you have only printed photos, please do not attempt to scan them yourself unless you have access to a high-quality scanner; simply send us the picture.

- Visual Media Waiver
Harvey Mudd College reserves the right to photograph and/or videotape students, faculty, staff and guests while on College property, during College-sponsored events or during activities where they are representing the College. These images and audio may be used by Harvey Mudd College for promotional purposes, including use in the College magazine, press releases, booklets, brochures, flyers, newsletters, advertisements, the College website and associated sites, and other promotional materials. This serves as public notice of the College’s intent to do so and as a release of permission to the College to use such images as it deems fit. If you should object to the use of your image, you have the right to withhold its release by filling out a form at the Office of Communications.

- Health Insurance Information
All students at HMC must have health insurance coverage. Students may be covered under an already existing policy, in which case they must submit an insurance waiver. If a student does not have a current health insurance coverge, they will need purchase the Claremont Colleges student insurance. More information regarding this insurance can be found here. The on-campus medical services provided by Student Health Services are available to all students regardless of their health insurance coverage.

- Student Health Services Information form
This pdf document includes a cover letter that explains the “Entrance Personal Health History/Medical Examination Report Form.” You must complete this form before you can use the Student Health Services. Some immunizations are also required. The document includes a letter explaining the risks of meningitis at college and recommends that incoming students get immunized against it. The final parts of the document include the Health History and Exam Form and an emotional health history form. Please note: Send this packet with all health exam and history forms directly to the Student Health Services at 757 College Way, Claremont, CA 91711 by August 1. - Ontario Airport Shuttle Reservation
Mudd runs a free shuttle van from the Ontario International Airport (ONT) to campus on the opening day of Orientation, Thursday, August 30, 2012. - Consent to Treatment of a Minor
If you will be under 18 years old, the date you arrive at Mudd, you must have your parent or guardian complete this form. Since hospitals cannot treat minors without the consent of a parent or guardian, this mandatory form allows you to receive emergency medical treatment in case your parents or guardian is not able to be reached.

- Final Grades / High School Transcripts
You must submit an official final high school transcript to the Admission office. Your admission and final enrollment at HMC is contingent on the successful completion of the Spring term.

- Housing Agreement (not yet available)
Each Harvey Mudd student living on campus must read and sign a housing agreement. After arriving on campus, the students will receive an e-mail informing them when the housing agreement is available.
