All deadlines for forms and documents are listed by due date, and should be returned to the Dean of Students office. Please allow several days to get these forms checked-in on our Received Checklist and distributed to the correct department. Deadline dates are nearly all uniform to make it easier to remember them: simply mark your calendar for the 10th of June, 10th of July, 1st of August and 10th of August as important deadlines. Note that the deadline for placement forms is slightly different, the 19th of July. There is an additional deadline, the 27th of August, for the Housing Agreement. - Roommate Matching Form
The Roommate Matching form is essential for you to complete thoroughly and honestly (no parents peeking). The Orientation Directors and the Dean of Residential Life read all forms very carefully and try to make a workable match of you and a roommate, as well as you and a dorm. The form is available through SurveyMonkey; both it and the more creative Sponsor Matching form are considered in matching roommates.

- Student & Family Information Form
The Student & Family Information form collects the same information previously collected by the Room & Board, Parent, and Emergency Data Information forms. The form is relatively short and should not take too much time to complete. It asks you for general information, family contact information, at least one emergency contact, and meal plan choice.

- Tuition Payment Agreement (PDF)
All incoming students are required to designate a tuition payment method. Please follow the link to view payment options. Please make sure to initial your payment choice and sign the form. Skip over the Student ID number portion since you will not have that until you arrive on campus.
Housing Deposit

The $150 housing deposit will be used as a room damage deposit and is separate from your admission enrollment deposit. Please make the check to Harvey Mudd College and mail with the Tuition Payment Agreement.
- Photos
Each year photos of new students are published in the Student Handbook. If you have not already submitted your pictures, please send two color or black-and-white wallet-size photos of yourself.

- Photo Release Form
A Photo Release Form is requested from every student to use your name, picture, portrait, biographical information or photograph in all forms and media and in all manners for public relations, advertising, or any other lawful purposes.

- Summer Institute
Harvey Mudd College Summer Institute (HMCSI) is an academic and personal enrichment program designed for underrepresented minorities in science, math, and engineering. This program is available for up to 20 first-year students. Information about this program can be found at the Summer Institute website.

- Advisor Matching Form
First-year students have the opportunity to establish first-hand relationships with their advisor and other students by participating in a workshop during Academic Orientation. This form is available through SurveyMonkey. You will need additional information about available advising workshops, which can be found here.

- Final Grades / High School Transcripts
You must submit an official final high school transcript to the Admission office. Your admission and final enrollment at HMC is contingent on the successful completion of the Spring term.

- Sponsor Matching Form
Sponsors, the students who organize Orientation, use the Sponsor Matching form to place you into compatible groups for Orientation. The form is available through SurveyMonkey; both it and the Roommate Matching form are considered in matching students with sponsors.

- Health Insurance Information
All students at HMC must have health insurance coverage. Students may choose to remain covered under an existing policy or to purchase insurance from the Claremont Colleges. More information about the two options is available here.

- Humanities Selection Form
During the first semester at HMC, all first-year students will be enrolled in a Humanities 1 course focused on reading, writing, and critical thinking skills. Each student must complete the Humanities Form indicating his or her preferences. This form is available through SurveyMonkey. You will need additional information about the available sections of Humanities 1, which is available here.

- Student Health Services Information form
This pdf document includes a cover letter that explains that the “Entrance Personal Health History/Medical Examination Report Form.” You must complete this form before you can use the Student Health Service. Some immunizations are also required. The document includes a letter explaining the risks of meningitis at college and recommends that incoming students get immunized against it. The final parts of the document include the Health History and Exam Form and an emotional health history form. The exam and history forms should be submitted directly to the Student Health Service at 757 College Way, Claremont, CA 91711 by August 1. The Student Health Service webpage is at http://www.cuc.claremont.edu/shs/.

- Ontario Airport Shuttle Reservation
Mudd runs a free shuttle van from the Ontario International Airport (ONT) to campus on the opening day of Orientation, Thursday, August 27, 2009. Please follow the link to reserve a space on the Ontario Airport shuttle.

- Consent to Treatment of a Minor
If you will be under 18 years old when you arrive at Mudd, you need to have your parents complete this form. Since hospitals cannot treat minors without the consent of a parent, this form allows you to receive emergency medical treatment in case they can’t contact your parents.
