Outstanding Alumni Nomination Form
Use this form to nominate someone for the Outstanding Alumni Award. Categories of recognition are:
- Community Service (work with institutions, the environment, organizations)
- Global Contribution (work in education, the environment, world relations, philanthropy)
- Science and Technology (technological developement and application, product development, advances in medicine)
To qualify, contributions should demonstrate a high level of sustained commitment involving considerable personal sacrifice. The size of the group effected does not matter, but the contribution must make a positive impact on humanity or society. The contribution might involve an organization, community or individual. The nominee is not required to have received prior recognition, honors or awards.
