Students:
In order to have a petition considered by the Scholarly Standing Committee, you must complete this form, in consultation with your advisor(s), and submit it to the Office of the Registrar at Platt Campus Center. Please see the table of meeting dates and petition deadlines for information on when your petition must be received. It is recommended that you copy and paste this form into a word processing document and then type in the necessary information.

PLEASE NOTE:  Your advisors' signatures are required below. A brief email message from an advisor to the Registrar may take the place of a signature. Petitions lacking a signature (or equivalent) will not be considered by the committee. Furthermore, petitions should be submitted to the Registrar, not to the committee chair or other members.
