Major Declaration
When you are ready to declare your HMC major (in your sophomore or early junior year), please submit a Major Declaration Form (PDF) to the Registrar's Office.

Change in Major or Advisor
A Change in Major or Advisor form (PDF) submitted to the Registrar's Office, is required when submitting a change in your major or advisor.

Off-campus Majors
When declaring an off-campus major, one of the following forms needs to be completed and turned into the Registrar's Office:
Off-campus major with a HMC minor
Off-campus second major

Enrollment/Degree Verification Requests
An Enrollment/Degree Verification Request form (PDF) must be submitted to the Registrar's Office for enrollment and degree verifications.

Biographical Information Updates
In order to keep your contact information up-to-date, please use the Biographical Information Update form (PDF) to notify the Registrar’s Office of any changes in your name, residence, phone number or emergency contact information.

Transfer Course Approvals
When requesting course credit transfers, a Transfer Course Approval form (PDF) needs to submitted to the Registrar's Office.

Summer Math Registration Form
In order to take math courses during summer at Harvey Mudd, the summer math registration form must be submitted to the Registrar's Office by the published deadline.  (For 2013, by April 12, 2013)
