When you are ready to declare your HMC major (in your sophomore or early junior year), please submit a Major Declaration Form (PDF) to the Registrar's Office.
Change in Major or Advisor
A Change in Major or Advisor form (PDF) submitted to the Registrar's Office, is required when submitting a change in your major or advisor.
When declaring an off-campus major, one of the following forms needs to be completed and turned into the Registrar's Office:
Off-campus major with a HMC minor
Off-campus second major
Enrollment/Degree Verification Requests
An Enrollment/Degree Verification Request form (PDF) must be submitted to the Registrar's Office for enrollment and degree verifications. For other requests such as diploma order, please use Special Request form.
Biographical Information Updates
In order to keep your contact information up-to-date, please use the Biographical Information Update form (PDF) to notify the Registrar’s Office of any changes in your name, residence, phone number or emergency contact information.
Add/Drop Form (pdf) - To be used ONLY in the event that the Student Portal is unavailable.
The Registrar’s Office will RESOLVE/OVERRIDE COURSE CONFLICTS permitting for student registration by way of the Student Portal. Students will still need to registrar for courses on Portal and obtain PERMission approvals when applicable (Course conflict approval does not override PERMissions and does not guarantee enrollment in a course). ALL instructors for the indicated conflicting courses must sign the Conflict Approval form.
Students must read the terms and conditions outlined on the Course Pass/Fail Approval and indicate to the agreement by including their Student Initial (located on the bottom left of the form).
Independent Study/Directed Reading/Research Approval forms MUST be completed by ALL PARTICIPANTS and submitted to the Registrar's Office for section assignment and processing (Independent Study/Directed Reading/Research indicated on the Registration card is for schedule planning reference only and WILL NOT PROCESSED by the Registrar's Office).
Overload Approval forms - Available through the Academic Affairs Office (Located in the Dean of Students Office in Platt Center).Transfer Course Approvals
When requesting course credit transfers, a Transfer Course Approval form (PDF) needs to submitted to the Registrar's Office.
