Key Request Form

Key requests should be made for new staff/faculty members and students needing key access. Requests may only be made by the department supervisor, chair, faculty member or department secretary.

(Note: List multiple names only if you are requesting the same key(s) for all recipients)

(Note: Expiration date MUST be filled out if you are requesting a key(s) for a student(s).)
 
   
Once your request has been processed and the keys are ready, the requestor will receive an email indicating that the key may be picked up.


Keys must be personally checked out to the individual who will carry the key, and their HMC ID card must be presented.
