Note 1:  Primary phone designation: In some instances messages will only be sent to the primary phone number. If permission to use your cell phone number for emergency notifications was given during registration (biographical data or updates), it will be used as the primary phone number unless changed by this form.

Note 2: Only ONE cell phone may be designated to receive text message alerts. If the cell phone number listed emergency notifications during registration (biographical data or updates), opted to receive Voice Only message alerts, an additional cell phone number (via this form) may be designated to receive a text message.
