Transfer Course Approvals
When requesting course transfers, please submit a Transfer Course Approval form (PDF).
Change in Major or Advisor
A Change in Major or Advisor form (PDF) is required when requesting a change in your major or advisor.
Off-campus Majors
When declaring an off-campus major, one of the following forms needs to be completed:
Off-campus major with a HMC minor
Off-campus second major
Enrollment/Degree Verification Requests
An Enrollment/Degree Verification form (PDF) must be submitted for enrollment and degree verifications.
Biographical Information Updates
In order to keep your academic records up-to-date, please use the Biographical Information Update form (PDF) to notify the Registrar’s Office of any changes in your name, residence, phone number or emergency contact information.
