* Primary phone designation: In some instances (i.e., non-emergency test mode) messages may only be sent to the primary phone number. If the cell phone number listed on the Registrar's Emergency Data Sheet is used for EMERGENCY NOTIFICATIONS, it will be used as the primary phone number unless changed by this form.

** Only ONE cell phone may be designated to receive text message alerts. If a cell phone number included on the Registrar's Emergency Data Sheet opted to receive Voice Only message alerts, an additional cell phone number (via this form) may be designated to receive a text message.
